Summary
A phase of striking change in an ongoing psychoanalytic psychotherapy gives occasion for clinical and theoretical reflections concerning the vicissitudes of the death instinct as elaborated by Klein. The hypothesis is developed that in the case discussed the re-introjection of the death instinct, which was the object of excessive projective identification, is of crucial importance in the transition from the paranoid-schizoid to the depressive position and integration. The case leads to some views on the importance of the inner fantasy world and its relation to external reality.
