Time and timelesness in adolescence Psychoanalytic psychotherapy in early psychosis
This paper examines how the developmental breakdown that is inherent in an incipient psychosis can be partially or completely reversed through psychoanalytic psychotherapy. The specificity of psychopathology in adolescence is discussed, also the fact that a psychotic breakdown is closely linked with the issues of adolescence. The author argues that because it is very difficult to differentiate between a severe adolescent crisis with psychotic symptoms and a structural psychosis, we have to give psychotherapy a chance. In the search for the meaning of an early psychosis a developmental perspective is integrated with a Kleinian-Bionian point of view. Important for a psychotherapeutic understanding is the fact that adolescence is responsible for a breaking point in infantile development because earlier events become loaded with a new, often traumatic meaning, what we call Nachträglichkeit. Throughout the paper much attention is given to Bions thesis that psychosis develops when reality is too painful to be contained and also to the fact that the therapist should not refrain from getting into contact with psychotic elements in the transference.
